Per the Iowa Department of Public Health, all students must be up to date on immunization in order to attend school.

Proof of up-to-date immunizations or Medical/Religion exemption waiver must be turned in prior to the first day of school.

Students will not be able to start/attend school if immunizations are not up to date.

  • 12th-graders must have 2 Meningococcal vaccines prior to starting school or 1 dose of vaccine given at 16 years of age or after.
  • 9th-graders are required to have a dental exam and a completed Dental Certificate turned into BHS prior to start of the school year.
  • 7th-graders must have one-time Tdap vaccine and a Meningococcal Vaccine.
  • Kindergartners need their DTap, IPV, MMR and Varicella Vaccines prior to starting school along with required Dental Certificate and proof of a Normal Lead Test at some point in their lifetime.

Recommended are the Physical Form completed and Vision Exam completed.

Please contact your student’s medical provider or Des Moines County Public Health to get your student updated on their immunizations as soon as possible. Please don’t wait as they may run out of the vaccine and your child would not be able to start school without these required vaccines.
